1. Complete STUDENT/APPLICANT INFORMATION and have Parent or Guardian Sign
2. Have Employer fill out PLEDGE OF EMPLOYER
3. Have Physician fill out PHYSICIAN’S CERTIFICATE FOR MINOR WORK PERMIT on opposite side

Bring completed form with copy of birth certificate to: BOARD OF EDUCATION

The student’s signature is required on the Work Permit; therefore, the student should return the application.
Each change of employment requires a new Work Permit.
Students 14 -18 years of age with a promise of a job are eligible for work permits.
